During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal inspection was initiated and completed on July 14, 2025. The on-site inspection began at 11:50am and ended at 1:45pm. The inspector reviewed compliance in the areas listed above. There were 12 children present and 2 caregivers; the point total was 32. The inspector reviewed 5 children?s records and 4 caregiver/household member records on-site. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on July 25, 2025. A POC submitted after this date will not appear on the public website.

A tuberculosis (TB) screening is required every two years from the last screening.

An updated TB screening for Household Member #1 was not completed and was overdue by seven weeks.
Plan of Correction: Household Member #1 was not in town to get the form but their form is in now
Standard 8VAC20-800-240-A
Areas and furnishings of the family day home, inside, shall be maintained in a clean, safe, and operable condition.

A room occupied by children was not in a safe condition when there were two hot wax warmers, within reach of children.
Plan of Correction: Wax Warmers were removed from the room immediately.
Standard 8VAC20-800-320-B
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

There were two low uncovered electrical outlets observed in an area occupied by children.
Plan of Correction: Outlets were immediately covered.
