Inspector Notes

An unannounced monitoring inspection was completed on 10/30/2024 as a part of the licensure period.
The on-site inspection began at 11:00am and ended at 12:45pm. The inspector reviewed compliance in the areas listed above. This inspection included: document review, tour of the facility, interviews, observations and measurements. The inspector reviewed 7 children?s records and 3 staff records. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-1
The center is required to provide for each staff person, a sworn statement or affirmation disclosing whether he has ever been convicted of or is the subject of pending charges for any offense within or outside the Commonwealth and whether he has been the subject of a founded complaint of child abuse or neglect within or outside the Commonwealth.

Staff #2 who has been employed for approximately 10 days did not have a sworn statement of affirmation.
Plan of Correction: This was an oversight. The director will have the employee sign the sworn statement by the end of the day and keep it in the record.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

There was a bottle of bleach water on the changing table in the 16-24 month-old room and a can of air freshener in in the changing area of the 3 year-old room.
Plan of Correction: The director will remind staff all chemicals are required to be kept in a locked place. She will monitor for compliance.
