Inspector Notes
An unannounced inspection was completed on 03/11/2025 in response to a complaint received by the licensing office on 02/25/2025 relating to allegations of Programs and Supervision. The inspector reviewed 2 children?s records and 5 staff records. This inspection included: document review, tour of the facility, interviews, observations and measurements. The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Standard 8VAC20-780-340-D
In each grouping of children at least one staff member who meets the qualifications of a program leader shall be regularly present. During snack, on 02/18/2025 the 16 month - 24 month classroom did not have at least 1 staff member who met the qualifications of a program leader. The program director confirmed Staff A and Staff B, whom were present at that time were teacher assistants and did not meet the qualifications of a program leader.
Plan of Correction: The Program Director will address the roles of Program Leader (Lead Teacher) and Teacher Assistant with the entire staff. She will also ensure each group of children are always in the presence of a staff that meets Program Leader qualifications/
Standard 8VAC20-780-400-A
Staff shall interact with children to provide needed help, comfort and support in order to promote the child's physical, intellectual, emotional, and social well-being and growth.
Neither Staff A or Staff B provided comfort to a crying 18 month-old (Child 1) who was sitting alone on the carpet away from his peers. Video footage confirmed Staff A dropped a blanket on top of Child 1 as she walked away with no other interaction. The blanket covered the child's entire body. Staff B, also in the room vacuuming, did not comfort or interact with the crying child.
Plan of Correction: Staff received disciplinary action and one was terminated. The director met with the entire staff to discuss the incident and review behavioral guidance and what to do in the event there is a child whom they can not comfort.
Standard 8VAC20-780-410-2
It is a forbidden action to enclose a child in a small confined space or any space that the child cannot freely exit himself.
1. During nap Staff B and C enclosed children in the 16 month-24 month classroom on their cots with a fitted sheet that covered their entire bodies, including the head. The sheet was secured on all 4 sides under the cot so the sheet could not come off.
2. Video confirmed an 22 month-old Child 2 was wrapped in a sheet covering his entire body and face, where he was unable to free himself. Staff stated this was to prevent the children from running or jumping off of their cots.
2. During meals, 16-18 month-old children are placed in high chairs to eat and are not taken out of the chairs until after all diapers have been changed and staff have cleaned the room. Staff stated it is to keep them from running around the classroom.
Plan of Correction: Staff received disciplinary action and one was terminated. The director met with the entire staff to discuss the incident and review behavioral guidance and actions that are forbidden.
Standard 8VAC20-780-560-L
Staff shall sit with children during meal times. On 02/18/2025 Staff in the 16-24 month-old room did not sit with the children during meal time. The children were placed in high chairs in front of a video while they cleaned.
Plan of Correction: The Program Director has removed screens from the classrooms. Staff are required to sit with the children during meals and she will monitor this for compliance.
