Inspector Notes

An unannounced code compliance inspection was conducted on6/27/2022 from 9:30am to 10:50am. At the time of entrance,74 children were in care with 14 staff members present. Children were observed doing the following: sitting in a large room for a group meeting, playing in centers, eating snack, making a fourth of July parade stick and coloring. The site was clean and organized. Interactions between the children and staff were positive. A selection of staff and children records were reviewed. Based on review of six staff records, the center did not have a completed sworn disclosure statement for one staff prior to date of hire.
Evidence:
1. Staff #4 (Date of hire 8/24/2020) had a signed Sworn Disclosure Statement, but it was not dated at the time of review.
Plan of Correction: We have created a second point person to review all employment documents to ensure all are probably filled out with signature and date.
