Inspector Notes
An unannounced renewal inspection was conducted today from 10:15 am-11:35 am. There were 8 children supervised by 3 direct caregivers. The home, 3 staff records, 5 children?s records, 3 household members? records, evacuation drills, emergency supplies, and policies were inspected. Children were observed engaging freely, eating snacks, and playing outside. Areas of non-compliance are identified in the Violation Notice. Thank you for your cooperation during this inspection. 6Evidence: The most recent criminal history result on file for Caregiver #1 is dated 6/13/2018 and expired on 6/13/2023.
Evidence: There was no documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance for Child #1 (start date 2/3/2023) and Child #2 (start date 2/13/2023).
Evidence: There was no documentation of Caregiver #3's orientation training. The start date for Caregiver #3 is 9/20/2022.
Evidence: Child #1 (start date: 2/3/2023), Child #2 (start date 2/13/2023), Child #3 (start date 5/30/2022), Child #4 (start date 2/22/2021), and Child #5 (start date 2/9/2022) records did not contain the family day homes policies for medication administration.
Evidence:
1. Child #3's last annual emergency contact review was on 1/28/2022.
2. Child #4's last annual emergency contact review was on 2/22/2021.
3. Child #5's last annual emergency contact review was on 2/9/2022.
