Inspector Notes
An unannounced monitoring inspection was conducted on 07/15/2022 between approximately 8:40 a.m. and 9:50 a.m. There were 8 children present with the Provider and 2 assistants for a total of 27 points within the supervision guidelines. The children were observed playing outside, doing tummy time and playing with cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, caregiver and household member records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 3Evidence: The TB screening form for Caregiver #4 (hire date: 06/17/2022) was completed on 02/04/2020.
Evidence: The last TB screening form for Caregiver #3 was dated on 02/28/2020.
Evidence:
1. Child #1 had two bottles in the refrigerator that were not labeled with the child?s full name and date.
2. Child #2 had one bottle in the refrigerator that was not labeled with the date.
