Inspector Notes

An unannounced monitoring inspection was conducted on 07/15/2022 between approximately 8:40 a.m. and 9:50 a.m. There were 8 children present with the Provider and 2 assistants for a total of 27 points within the supervision guidelines. The children were observed playing outside, doing tummy time and playing with cars. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children, caregiver and household member records, fire drill log, menu, attendance and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the provider did not obtain from each caregiver at the time of hire a Report of Tuberculosis (TB) screening form that was completed within the last 30 days.

Evidence: The TB screening form for Caregiver #4 (hire date: 06/17/2022) was completed on 02/04/2020.
