Inspector Notes

An unannounced monitoring inspection was conducted today between the hours of 2:50pm and 3:48 pm. There were 7 children (17 points) directly supervised by the provider and one assistant caregiver. The home, 2 caregiver records, 5 children?s records, 1 household member?s record, evacuation drills, and emergency supplies were inspected. Children were observed waking up from nap and having snack. Handwashing procedures were also observed. Areas of non-compliance are identified in the Violation Notice.

Based on record review, 1 of 5 children's records did not contain all required information.

Evidence:
1. The record for Child #2 did not contain the address for 1 of 2 designated people to call in an emergency if a parent cannot be reached
Plan of Correction: The provider will ask the parents for the information.
Standard 8VAC20-800-80-C
Based on record review, 5 of 5 children's records did not contain complete documentation of proof of identification for each child in care.

Evidence:
1. The records for Child #1, #2, #3, #4, and #5 did not contain documentation of date and person verifying original proof of identification.
Plan of Correction: Provider will update the records today.
Standard 8VAC20-800-90-A
Based on record review, the provider did not obtain documentation that one child has been adequately immunized before the child's first date of attendance.

Evidence:
1. Child #2 began attending the family day home on 11/21/2022.
2. The immunizations in the record for Child #2 were dated 12/13/2022.
Plan of Correction: Immunizations will be obtained before a child's first day.
