Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews) and a virtual tour of the program. A monitoring inspection was initiated on2/3/2022 and concluded on 2/3/2022 The provider was contacted by telephone and a virtual inspection was conducted. There were 9 children present, ranging in ages from 14 months to 4 years, with 3 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
Based on a review of children records, the provider did not obtain documentation of a physical examination within 30 days after the first day of attendance.
Evidence: Child #1, first attendance date 8/23/2021, does not have documentation of a physical examination.
Plan of Correction: The parent will be contacted today to obtain the child's physical exam.
Standard 8VAC20-800-90-A
Based on record review, before a child may attend, the provider did not obtain proper documentation that the child has been adequately immunized.
Evidence: Child #1, date of first attendance 8/23/2021 has documentation of the immunization but the form was not signed or dated by the physician.
Plan of Correction: The parent will be contacted today to have the form signed by a physician.
