Inspector Notes

An unannounced monitoring inspection was conducted on 2/29/2024 from 9:35 am to 10:30 am. There were 2 children supervised by 1 direct caregiver. The home, 2 caregiver's records, 3 children?s records, 1 household member's record, evacuation drills, and policies were inspected. Children were observed engaging freely. Areas of non-compliance are identified in the Violation Notice. Please submit your plan of correction within 5 business days for it to be included on the website. Based on observation and interview, the provider did not ensure that child-resistant protective covers shall be installed on all unused electrical outlets and surge protectors accessible to children under five years of age.

Evidence: There is an electrical outlet on the wall that does not have a protective cover above the couch in the downstairs playroom.

Based on record review and interview, not all children's records contain general written permission shall be obtained from the parent of each child for the provider to take the child off the premises of the family day home.

Evidence:
1. Child #1, Child #2, and Child #3's records did not contain general written permission for walking.
2. Caregiver #1 stated that they walk the children to a neighborhood playground.
