Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews, and a tour of the program.

A monitoring inspection was initiated on 1/24/2022 and concluded on 2/2/2022. The director was contacted by email to initiate the inspection. There were 61 children present and 8 staff. The Inspector reviewed 4 children?s records and 4 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and interview it was determined the center did not have annual approval from the health department.
Evidence:
1. The health department inspection available was dated 10/10/2020.
2. Staff confirmed there was not an updated health inspection available during inspection.
Plan of Correction: Center will request an updated health department inspection.
