Inspector Notes

A monitoring inspection was conducted on 8/21/2023 from 11:20 am to 12:45 pm. Seven children, ages 5 months to 4 years (total of 183 points) were in care with two caregivers. The Licensing Inspector reviewed four children?s records, one provider record, two staff records, and other program records. The children were observed having a bottle and napping.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-800-90-A
Based on child record review and interview, the provider failed to obtain an affidavit stating that the administration of immunizing agents conflicts with the parent?s or child?s religious tenets or practices prior to beginning attendance.

EVIDENCE:
Child 1 began attending on 9/7/2022, and the child?s record does not have documentation of immunizations. Staff 1 stated that the family has a religious objection to immunizations, and there is no affidavit in the record for a religious exemption from immunizations.
Plan of Correction: The provider will contact the parents to get the affidavit completed. The child will not attend until the document is obtained.
