Inspector Notes
An unannounced monitoring inspection was conducted on March 23, 2023 from approximately 8:30 am-10:06 am. There were 23 children were being picked up by the school bus ,38 children
present in the building during the inspection with 10 staff members. Additional staff was on site for administrative and support services. Children were observed freely playing,eating morning snack, outside playing, diapering and transitioning from classrooms. 5 staff records and four children records were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.
Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: Child #3 start date 09/08/2022 did not have documentation of physical. Staff confirmed and contact parent and recieved during the inspection.
Evidence:
Staff #1 was hired 02/09/2023 her most recent TB test is 10/13/2022. The TB test was not completed with in the last 30 calendar days of date of employment.
Evidence:
In the toddler classroom , the diaper surface had a 1 inch tear, which causes the surface to be absorbant.
Evidence: In the infant room, there was two infants that did not have protective belt fastened securley.
