Inspector Notes

An unannounced monitoring inspection was conducted on March 23, 2023 from approximately 8:30 am-10:06 am. There were 23 children were being picked up by the school bus ,38 children
present in the building during the inspection with 10 staff members. Additional staff was on site for administrative and support services. Children were observed freely playing,eating morning snack, outside playing, diapering and transitioning from classrooms. 5 staff records and four children records were reviewed during the inspection. Licensing Inspector reviewed indoor areas, supervision, activities, equipment, emergency evacuation and shelter-in-place drill documentation, emergency procedures and required posting.

Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-B
Based on staff interview and record review, the licensee did not ensure before the child's attendance; or within 30 days after the first day of attendance.

Evidence: Child #3 start date 09/08/2022 did not have documentation of physical. Staff confirmed and contact parent and recieved during the inspection.
Plan of Correction: The center contacted mother during the inspection the physical was reviewed during the inspection.
Standard 8VAC20-780-160-A
Based on record review and staff interview, the licensee did not ensure the documentation have been completed within the last 30 calendar days of the date of employment.

Evidence:
Staff #1 was hired 02/09/2023 her most recent TB test is 10/13/2022. The TB test was not completed with in the last 30 calendar days of date of employment.
Plan of Correction: The TB test will be cimpleted with in 30days prior to employment.
Standard 8VAC20-780-500-B
Based on observation, the licensee did not ensure a nonabsorbent surface for diapering or changing should be used.

Evidence:
In the toddler classroom , the diaper surface had a 1 inch tear, which causes the surface to be absorbant.
Plan of Correction: The diapering surfces will be replaced by 03/31/2023.
Standard 8VAC20-780-570-A
Based on observation and staff interview, the licensee did not ensure when a child is placed in an infant seat or high chair, the protective belt shall be fastened securely.

Evidence: In the infant room, there was two infants that did not have protective belt fastened securley.
Plan of Correction: Staff will be informed immedatley that straps in fastened all times.
