Inspector Notes

A renewal inspection was initiated and concluded on 10/11/2023.There were 60 children present with 12 staff supervising. The children were observed during outside play time, lunch time and nap time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 11:05 am and concluded at 1:05 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 8VAC20-780-140-B
(Repeat Violation) Based on staff interview and record review, the center did not ensure each child shall have a physical examination by or under the direction of a physician before the child's attendance; or within 30 days after the first day of attendance.

Evidence: Staff confirmed that Child #1, start date 8/15/2022, did not have documentation of a physical.
Plan of Correction: Will contact parent and get the child's physical.
Standard 8VAC20-780-40-M
Based on observation and staff interview, the center did not ensure the current list of children's allergies, sensitivities, and dietary restrictions shall be dated and kept confidential in each room or area where children are present.

Evidence: The preschool 3 room had an allergy list hanging on the wall. The list was not kept confidential as staff confirmed that parents are allowed in the rooms when dropping off and picking up their children.
Plan of Correction: Will cover the list or come up with another way to keep it accessible to staff but confidential.
Standard 8VAC20-780-570-H
Based on observation and staff interview the center did not ensure that prepared baby food not consumed during that feeding by an infant may be used by that same infant later in the same day, and it shall be discarded by the end of the day or returned to the parent.

Evidence: In the pre-toddler room staff confirmed that the bottle labeled with child #1's name was dated 10/9/2023 and was not discarded or returned to the parent at the end of the day.
Plan of Correction: The bottle was removed at the inspection.
