Inspector Notes

A monitoring inspection was initiated and concluded on 4/9/2024.There were 68 children present with 14 staff supervising. The children were observed during classroom time engaged in various centers and observed during teacher led activities and outside time. A total of 5 child records and 5 staff records were reviewed. The inspection started at 10:20 am and concluded at 11:50 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on record review and staff interview the center did not obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center.

Evidence: The director confirmed that Child #3 (start date 4/1/2024) did not have documentation of immunizations.
Plan of Correction: Child has doctor appointment scheduled for April 17. Will obtain complete physical at that visit with shot records included.
