Inspector Notes
An unannounced, on-site focused inspection was initiated on 4/23/2025 and completed on 4/23/2025 The on-site inspection began at 10:00am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. This inspection included document review and interviews.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Violations
1On 4/10/2025, Staff #1 attempted to carry Child #1 (age 7 months) inside from the playground while in a booster seat. The child was not buckled in and the booster seat broke. Child #1 fell to the ground from approximately hip height to Staff #1. The warning label on the seat specifically stated to "never lift or carry child in booster seat".
