An unannounced, on-site focused inspection was initiated on 4/23/2025 and completed on 4/23/2025 The on-site inspection began at 10:00am and ended at 10:30am. The inspector reviewed compliance in the areas listed above. This inspection included document review and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

When staff are supervising children, they shall always ensure their care, protection, and guidance.

On 4/10/2025, Staff #1 attempted to carry Child #1 (age 7 months) inside from the playground while in a booster seat. The child was not buckled in and the booster seat broke. Child #1 fell to the ground from approximately hip height to Staff #1. The warning label on the seat specifically stated to "never lift or carry child in booster seat".
