Inspector Notes

An unannounced, monitoring inspection was initiated on and completed on 12/16/2025. The on-site inspection began at 1:00pm and ended at 3:20pm. The inspector reviewed compliance in the areas listed above. There were 65 children present and 10 staff. The inspector reviewed six
children?s records, and six staff records on site.

This inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations

Standard 8VAC20-780-60-A-8
A written care plan is required for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and the steps to be taken in the event of a suspected or confirmed allergic reaction. Child #6, who has been enrolled for eight months, has a diagnosed food allergy and did not have a written care plan in their file.
Plan of Correction: Family members were contacted and an allergy action plan will obtained if the food allergy is still applicable.
