Inspector Notes
An unannounced renewal inspection was conducted on 11/9/2023 from 8:55 to 10:40.
The provider and two assistants were present with 11 children, totaling 31 points. The children were observed arriving for the day, playing and doing art projects. Lots of interaction between caregivers and children. The provider has lots of materials and space for the children in care. She is serving stew and bananas, blueberries and apples today for lunch
Discussed the administrative part of being licensed, although the provider has great programming and a very caring environment, she has a hard time keeping up with the administrative part of being licensed. After last inspection she was offered assistance from the LI but declined. Several violations related to the administrative part of being licensed was cited at this inspection. One repeat citation. A violation notice was given to the provider along with this inspection summary. Reviewed 4 household member records, 3 staff records and 4 children's records during this inspection.

6Evidence: Four out of five household members have criminal records checks that are more than 5 years old. Household member 1,3 and 5.
Evidence: Based on the review of records, household member 1,2,3 and 5 are missing the subsequent child protective service check.
Based on the review of staff records, the provider failed to get some background checks required for staff.
Evidence: The two assistants are missing the central registry check.
Staff 2, was missing the check at the last inspection, DOH 3/1/23.
Staff 3 is missing the check and has been working at the providers more than 30 days.
DOH 4/21/23
Evidence: According to the provider she has been taking English classes but no classes related to caring for children.
Evidence: Reviewed of 4 children's records, Child1, is missing the physical examination and immunization records. Child 4 is missing addresses for the emergency contacts.
Evidence: The diaper pad has several small holes that makes the pad absorbent when sanitized.
