Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 10/01/24 and concluded on the same day., as a part of licensure period. The on-site inspection began at 10:40 and ended at 12:00. The inspector reviewed compliance in the areas listed above. There were 11 children present and 3 caregiver; the point total was 30. The inspector reviewed 4 children?s records and 4 staff records . This inspection included document review, a tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-180-A
Each caregiver must have a subsequent TB screening done evert two years.

A household member TB screening was last done 4/19/22 and has not been updated.
Plan of Correction: I will send the household member to get the TB screening done.
Standard 8VAC20-800-60-B
A child's records shall have two emergency contacts.

The review of 4 children's record, resulted in two records, Child 1 and 3, missing the second emergency contact.
Plan of Correction: I will ask the parents for this information this evening.
