Inspector Notes

An unannounced, on-site renewal inspection was initiated on 4/23/25 and completed on the same day, as a part of licensure period. The on-site inspection began at 10:10 and ended at 11:30. The inspector reviewed compliance in the areas listed above. There were 8 children present and 3 caregivers ; the point total was 23. The inspector reviewed 4 children?s records and 4 caregiver/household members records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination by or under the direction of a physician prior to a child's attendance or within 30 days after the first day of attendance.

One child out of 5 children's files reviewed is missing the physical examination and has been in care for more than 30 days.
Plan of Correction: I will ask the parent to bring me the physical examination record.
Standard 8VAC20-800-180-A
The provider shall obtain for each caregiver and adult household member a current Tuberculosis screening and such screening must be repeated every two years from the date of the first screening.

A household member needs a subsequent TB screening done, this is a repeat violation.

Plan of Correction: I will make sure I send them both to get the TB screenings done.
Standard 8VAC20-800-60-B
The child's record shall contain the following information.

2 out of 5 children's records reviewed were missing items in the file.
Child 1 and 2 were missing the emergency medical authorization as well as proof of ID.
Child 2 was also missing the liability insurance information.
Plan of Correction: i will make sure I get all the information from the parents for the two children.
