Inspector Notes
An unannounced Monitoring Inspection took place on 10/06/2022 between the hours of approximately 9:10 a.m. and 12:50 p.m. There were 4 classrooms observed with a total of 45 children with 10 staff within the supervision guidelines. The children were observed playing with blocks and musical instruments, doing tummy time, playing on the playground and other organized activities. Positive interaction between staff and children were observed. A complete inspection of the physical plant, children and staff records, fire drill log, and emergency procedures were observed during this inspection. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. 6Evidence:
1. Staff #2 (start date: 05/12/2021) has resided out of state in the preceding five years. An out-of-state criminal history record check was not obtained.
2. Staff #4 (start date: 08/26/2020) has resided out of state in the preceding five years. An out-of-state criminal history record check was not obtained.
Evidence: Staff #4?s most recent tuberculosis screening on file was dated 03/16/2020.
Evidence:
1. There were two spots of chipped purple paint located near the window in the ?Penguins? classroom.
2. There were five spots of chipped pink paint located on the half wall in the ?Lambs? classroom.
3. There was a broken piece of the vinyl gate with sharp edges, located towards the bottom of the gate door on the playground.
Evidence:
1. At the time of inspection, a crib occupied by Child #5 and a crib occupied by Child #6 were approximately 4 inches apart.
2. At the time of inspection, a crib occupied by Child #6 and a crib occupied by Child #7 were approximately 2 inches apart.
3. At the time of inspection, a crib occupied by Child #3 and a crib occupied by Child #8 were approximately 1 inch apart.
Evidence:
1. During the inspection, Child #2 was observed drinking a bottle while in their crib.
2. During the inspection, Child #8 was observed drinking a bottle while in their crib.
Evidence:
1. Child #1?s record indicates an allergy to dairy. There was no allergy care plan on file.
2. Child #4?s record indicates an allergy to eggs. There was no allergy care plan on file.
