Inspector Notes
An unannounced inspection was initiated and completed on October 15, 2025, in response to a complaint received by the licensing office on October 8, 2025, relating to allegations of Staff Qualifications and Training, Staffing and Supervision and Special Care Provisions and Emergencies. The Department issued a Special Order on April 15, 2025, to place the licensee on probation for a period of six months; and contact parents, guardians, or other responsible persons regarding health and safety violations as a result of high-risk violations. The inspector reviewed 0 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation does not support the allegations; therefore, the complaint is determined to be not valid. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.
