Inspector Notes
An unannounced monitoring inspection was conducted on April 13, 2022 There were 9 children directly supervised by 3 staff. The childcare area, 4 staff records, 3 children?s records, evacuation drills, emergency supplies, and policies were inspected. There were no medications on site today. Children were observed settling down for nap. Diapering procedures were also observed. There was an assortment of age appropriate materials for children. The child care area was clean and organized. Areas of non-compliance are identified in the Violation Notice.

There were not central registry background check results for Staff 2 and Staff 3.
Evidence:
There was no documentation of a a physical examination by or under the direction of a physician for Child 1. Child 1's date of enrollment is documented as 1/3/2022.
Evidence:
1. Staff 1's record did not contain all required background checks.
2. Staff 2's record did not contain verification of age, job title, date of employment, emergency contact information, all required background checks, education, or reference checks.
3. Staff 3's record did not contain a current Tuberculosis screening, documentation that references were checked, or documentation of education.
4. Staff 4's record did not contain all required background checks, or documentation of education.
Evidence:
Staff 3's most recent Tuberculosis screening was conducted on 2/11/2019.
Evidence:
On the counter in the kitchenette off of the child care area there was an electric kettle that was plugged in and extremely hot to the touch. The door of the kitchenette was open and the area was accessible to children in care.
Evidence:
1. Child 1's record did not contain the child's health insurance policy number, the phone number for 1 of the 2 required emergency contacts, no documentation that proof of the child's age and identity have been verified, a record of immunization, or a physical conducted by a physician.
2. Child 2, and 3's record did not contain verification that proof of the child's identity had been viewed.

There was not an evacuation drill documented for 12/2021.
Evidence:
There was no documentation of immunization present in the record for Child 1.
Evidence:
No inspection notice was observed in the facility on the date of inspection.
