During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated on 10/22/2024 and completed on 10/22/2024. The on-site inspection began at 10:20am and ended at 11:20am. The inspector reviewed compliance in the areas listed above. There were 9 children present and 2 caregivers; the point total was 26. The inspector reviewed 6 children?s records and 4 caregiver/household members records [on-site] and [electronically on 10/22/2024].
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/31/2024. A POC submitted after this date will not appear on the public website.
Violations
13Child 2 and 4's record did not contain documentation of a physical examination.
On the date of inspection Caregiver 2 was acting in the capacity of a substitute in the absence of the provider. No record documenting the time of arrival was available for review.
In addition to satisfactory completion of first aid training and CPR training, caregivers shall obtain a minimum of sixteen clock hours of training annually in areas relevant to their job responsibilities.
Caregiver 3's record did not contain documentation of annual training.
The supplemental page from the two previous licensing inspections were posted in the facility. The supplemental page contains confidential information about children in care are state that they are not to be posted.
A small trampoline was observed in the backyard in the play area used by children.
1. Children from birth through 15 months of age count as four points each;
2. Children from 16 months through 23 months of age count as three points each;
3. Children from two through four years of age count as two points each.
Upon the inspector's arrival, Caregiver 3 was observed talking on the phone in the front yard. Caregiver 3 entered the family day home at the same time as the inspector, where Caregiver 2 was observed caring for 9 children alone (26 points).
Children's records shall be maintained with all required information.
Child 6's record did not contain the name, address, and phone number of two emergency contacts. The one listed emergency contact is the child's parent or the child's medical insurance carrier and policy number.
One child's hands were not washed after receiving a diaper change.
Child 6's record did not contain documentation that they have been informed of:
Operating information including the hours and days of operation, holidays or other times closed, and the telephone number where a message can be left for a caregiver; Schedule of fees and payment plans; Check in and check out procedures; Whether or not there is liability insurance of at least $100,000 per occurrence and $300,000 aggregate in force on the family day home operation as required by ? 63.2-1809.1 of the Code of Virginia; Requirement for the family day home to notify the parent when the child becomes ill and for the parent to arrange to have the child picked up as soon as possible if so requested by the home; Requirement for the parent to inform the family day home within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life- threatening diseases, which must be reported immediately; Requirement for paid caregivers to report suspected child abuse or neglect according to ? 63.2-1509 of the Code of Virginia;Custodial parent's right to be admitted to the family day home any time the child is in care as required by ? 63.2-1813 of the Code of Virginia; General daily schedule that is appropriate for the age of the enrolling child; Policies for the provision of food; Presence of a pet or animal in the home; Discipline policies including acceptable and unacceptable discipline measures; The name of the adult assistant or substitute provider; Provisions of the family day home's emergency preparedness and response plan; and
Address of the website of the department, with a note that a copy of this regulation and additional information about the family day home may be obtained from the website, including compliance history that includes information after July 1, 2003.
Child 3's medication authorization was authorized for the dates 8/13/2024 through 2/13/2025, but did not contain an additional authorization from the child's physician.
Two medications were stored in a child's cubby in an area accessible to children.
Child 1, 4, and 5's record did not contain documentation that the child's age and identity were verified.
Child 4's record did not contain documentation of immunization.
