Inspector Notes

The Licensing Inspector initiated and completed a monitoring inspection on 12/05/2022. The inspector was on site from 10:00am until11:45am.

There were twenty-five children under the care of eight staff. The ages of the children ranged from nine months to four years old. The children were observed during indoor and outdoor play, lunchtime and during a fire drill. Three children?s record and three staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program

Based on review the center failed to provide an Out of State Sex Offender Registry on individuals who have resided out of state within the past five years.

Evidence: Staff #1 who was hired on 10/13/2022 had no Out of State Sex Offender Registry on file.
Plan of Correction: Will get background check sent by 12/16/2022.
