Inspector Notes

An unannounced monitoring inspection was conducted today between 12:25pm-1:20pm. There were 10 children present with the Provider and 2 assistants for a total of 29 points within the supervision guidelines. The children were observed taking a nap. Positive interactions were observed during this inspection. A complete inspection of the physical plant, children and staff records, evacuation drill log, menu, attendance and emergency procedures were observed during this inspection. Violations were found during today?s inspection and can be viewed on the Violation Notice. Thank you for your cooperation. Based on a review of children's records, 1 out of 4 children's records did not have emergency contact information updated annually as required in 8VAC20-800-60 B 2 to ensure the information is correct.

Evidence:
1. Child #1 (doe: 1/7/2022) had emergency contact information that was last updated on 4/18/2023. It has been more than 1 year since the last update and it is required to be updated annually.
Plan of Correction: I will ask the parents to review it and sign it today.
