Inspector Notes
An unannounced complaint inspection was conducted on 06/05/2024 in response to a complaint received on 05/22/2024 regarding the following: administration; preventing the spread of disease; areas and equipment of the center; and behavioral guidance. There was a total of 63 children and 10 staff members present during the inspection. The inspector interviewed four staff members, reviewed 2 staff records and 2 children?s records during the investigation. The inspector arrived at 9:00 am and departed at 12:15 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.
The information gathered pursuant to an investigation of the complaint does support the allegation and the complaint was determined to be valid.
Violations
4Based on record review, the center failed to obtain documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with children.
Evidence: The record for Staff 5 did not contain the results of a tuberculosis screening. Staff 1 verified that Staff 5 worked at the center on 05813/2024.
Based on staff interviews, the center failed to ensure that timeout is not used with infant or toddlers.
Evidence: Staff 1 stated that they placed Child 1 (21 months old) in a crib for a two minute timeout because they hit another child.
Based on staff interviews, the center failed to ensure that actions towards children that are forbidden by the standards (confining children in a space that they cannot freely exit themselves; using equipment for the intended purpose) are not used.
Evidence: Staff 1 stated that they placed Child 1 (21 months old) in a crib for a two minute timeout. The crib was not used for its intended purpose when the child was place in it for time out. Child 1 could not freely exit the crib themselves.
Evidence: The record for Staff 5 did not contain the following information: documentation that two or more references as to character and reputation as well as competency were checked before employment (there was only one reference in the record during the 06/05/2024 inspection), Staff 1 verified that Staff 5 worked at the center on 05/13/2024; and date of separation from employment (Staff 5 is no longer employed at the center).
