Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 12/10/2024 and completed on 12/10/2024. The on-site inspection began at 12:25 pm and ended at 2:10 pm. The inspector reviewed compliance in the areas listed above. There were 27 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records on-site.

The inspection included document review, tour of the facility, interviews, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-340-F
Children under 10 years of age shall always be within actual sight and sound supervision of staff.

The infant class was observed during the designated rest time. The door to the classroom was closed, the lights were off, and the windows were covered with blankets to darken the room. The 2 staff were unable to maintain sight supervision of the 5 infants (ages 3 months-) in care in these conditions.

The toddler class was observed during the designated rest time. The door to the classroom was closed, the light was off and there are no windows in the classroom, creating an environment of complete darkness. Staff 2 was unable to maintain sight supervision of the 4 children in the room in these conditions.

At approximately 12:25 pm, Staff 4 was observed brining the leftover lunch upstairs to the kitchen from the downstairs Pre-K classroom. It was determined that Staff 4 was the only staff member supervising the Pre-K class that day. During the time that Staff 4 was upstairs, the 10 children in the pre-k classroom were not within actual sight and sound supervision of staff.
Plan of Correction: Will review the rest time procedures and review with the staff.

REPEAT VIOLATION

For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.

There were 10 children observed in attendance in the Pre-K room, but the attendance record indicated that there were 11 children in care.
Plan of Correction: Will make sure that the staff mark the attendance document when children move to different classes.
