Inspector Notes

An unannounced, on-site monitoring inspection was conducted on 8/8/2024, as a part of the licensure period. The on-site inspection began at 10:45am and ended at 11:45am. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 30. The inspector reviewed 4 children?s records and 2 caregiver records on-site on 8/8/2024. This inspection included document review of posting requirements, policies and procedures, emergency drill logs, authorization forms, emergency supplies, interviews and observations.

Based on record review and interview, the provider did not have written parent authorization for topical skin products noting any known adverse reactions obtained at least annually.
Evidence:
Parent authorizations for topical skin products expired for Child #2-5/2024, Child #3-2021 and Child #4-2/2024.
Plan of Correction: Provider to obtain updated written permission for topical ointments. Provider will review authorizations with parents annually.
