During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 4/14/2025, as a part of the licensure period. The on-site inspection began at 10:50am and ended at 12:15pm. The inspector reviewed compliance in the areas listed above. There were 11 children present and 2 caregivers; the point total was 27. The inspector reviewed 5 children?s records and 2 caregiver's records on-site.
This inspection included: document review, tour of the facility, interviews, and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-800-100-A
The provider shall obtain documentation of a physical examination prior to a child's attendance or within 30 days after the first day of attendance.

There was no physical exam records available for Child #1, # who has been in care for over 6 months.
Plan of Correction: Provider will request physical from parent today. For children enrolled in the future, the provider will review all documents within 14 days of child's first day to ensure all required information is obtained in a timely manner.
Standard 8VAC20-800-430-B
Accordion expansion gates shall not be used as protective barriers at stair openings.

Bottom of indoor stairs had accordion expansion gate as protective barrier.
Plan of Correction: Provider will order and install appropriate gate/barrier by 4/30/2025.
Standard 8VAC20-800-60-B
A child's record shall contain name, address and telephone number of two designated persons to contact in case of an emergency if the parent cannot be reached and names of persons other than the custodial parents who are authorized to pick up the child and written authorization for emergency medical care should an emergency occur and the parent cannot be located immediately unless the parent presents a written objection to provision of medical treatment on religious or other grounds.
