Inspector Notes
A monitoring inspection was conducted on 07/28/2022 between the hours of approximately 2:45 pm and 4:30 pm. There were 79 children present, ranging in ages from 4 years to 13 years, with 15 staff directly supervising. Children and staff were observed while doing yoga, rock climbing, doing crafts, and playing an organized game in the basketball court.
The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 4 child records and 4 staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Violations
6Evidence:
1) The record for Staff #4 (start date06/04/2022) did not contain documentation of having completed orientation training.
Evidence:
1) The orientation training documentation in the records for Staff #1, Staff #2 and Staff #3 did not contain the date when the orientation training was completed.
Evidence:
1) Sunscreen does not need to be kept locked but shall be inaccessible to children under five years of age: On the date of inspection, various plastic bins with sunscreen inside were observed on the floor of the gym, along one of the walls. One staff person stated that the youngest child enrolled in the "Robots" group was 4 years-old.
2) Sunscreen shall be in the original container and labeled with the child's name: 24 out of 48 sunscreen bottles were not labeled with the child's name.
Evidence:
1) On the date of inspection, emergency numbers were not posted.
2) The program director acknowledged not having the emergency numbers posted as required.
Evidence:
1) At least 3 lunchboxes and food containers were not dated and labeled with the child's name.
Evidence:
1) The record for Staff #4 did not contain the date when a second reference of character and reputation was checked.
2) References of character and reputation need to be checked before hire. The dates documented in the records of three staff persons show that references were checked after hire: *Staff #2 (hire date 06/09/22, references 06/10/22), *Staff #3 (hire date 06/06/22, references 06/9/22) and *Staff #4 (hire date 06/04/22, reference 06/07/22).
