During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 08/04/2025, as part of an Intensive Plan of Correction (IPOC). The program was required to provide an IPOC due to the serious nature and/or repetition of violations found during the last licensure period.

The on-site inspection began at 12:00 pm and ended at 1:30 pm. The inspector reviewed compliance in the areas listed above.

There were 51 children present and 8 staff. The inspector reviewed 1 child record and 9 staff records on-site.

This inspection included document review and tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within 5 business days from today, which will be the close of business on 08/12/2025. A POC submitted after this date will not appear on the public website.

When medication expires, the parent shall be notified that the medication needs to be picked up within 14 days.

Medication in the center for Child #1, has been expired for four weeks.
Plan of Correction: Contacting Child #1's parents to let them know to pickup medication.

*REPEAT VIOLATION*
When food is brought from home, the food container shall be clearly dated and labeled in a way that identifies the owner.
On the date of inspection, various lunchbags were either not labeled, not dated or both.
Plan of Correction: Swapped from tape to printer labels that are pre-dated and have parents put name on.
