Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, and a virtual tour of the program.

A renewal inspection was initiated on 09/16/2021 and concluded on 09/16/2021. The director was contacted by telephone and a virtual inspection was conducted. There were twenty-two children present, ranging in ages from 16 months to four years, with three staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of children's records, the center failed to obtain documentation of immunizations before the first day of attendance.

Evidence:
1. Child #1's immunization record was dated 08/24/2021. First date of attendance was 07/20/2021.
2. Administration verified the immunizations records were not obtained by the first day of attendance.
Plan of Correction: Administration will make sure that they get the immunization records before new children begin at the center.
