Inspector Notes

An unannounced renewal inspection was conducted beginning at 1:30 p.m. and ending at 2:30 p.m. There were 20 children in care ages 16 months through 5 years old under the supervision of 2 staff. Childcare areas, supplies and materials were observed. Children were observed during rest time. One additional staff was observed arriving at the end of the inspection. The outdoor play area was observed.

Six children's records were reviewed. There are no new staff. Staff training hours were reviewed. Emergency drills are posted and up to date. Emergency supplies were available including a full first aid kit.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of documentation, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years. Evidence: Child #4, age 16 months at enrollment, last documented immunization was dated 11/1/2021 and should have been updated before April of 2022. Child #5, age 16 months at enrollment, last documented immunization was dated 11/2022 and should have been updated by May of 2023.

Based on review of six children's records, the center failed to ensure each child's record contained required information. Evidence: Child #4, first day of attendance 12/13/2021, do not have a full address for one emergency contact. Child #5, first day of attendance 12/06/2022, did not have a second emergency contact.
