During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site focused inspection was initiated and completed on 4/03/2025, as a part of the provisional licensure period. The on-site inspection began at 10:12am and ended at 10:53am. The inspector reviewed compliance in the areas listed above. There were 10 children present and two caregivers; the point total was 26. The inspector reviewed three children?s records and two caregiver/household members records on-site on 4/03/2025.
This inspection included document review, tour of the home, and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.


Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/10/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
'Areas and furnishings of the family day home, inside and outside, shall be
maintained in a clean, safe, and operable condition.

The first wood deck step near ground level had three rusted nails sticking out of the splintered wood step.
Plan of Correction: I already contacted the contractor for the repair the step, but the fence needs HOA approval which will take time.

Poisonous materials shall be stored in an area inaccessible to children.

In the bathroom that children use, Lysol and Glade spray were on the bathroom floor, a container of Bleach was on top of the bathroom tub and toothpaste was on the sink near the faucet.
Plan of Correction: Relocated all Poisonous materials to a secure storage area that is completely inaccessible to children (Laundry cabinet with lock.)

Repeat Violation
Child-resistant protective covers larger than 1-1/4 inches in diameter shall be installed on all
unused electrical outlets and surge protectors accessible to children under five years of age.

Two unused electrical outlets were not covered with child-resistant protective covers and were
accessible to children under five years of age in the play room.
Plan of Correction: This is something I've been discussing with my assistant. She always forgets to put cover back after using it. We need to check every two hours to ensure that all the outlets have their protective covers on. make it a habit.

The provider shall ensure that a caregiver does not exceed 16 points by using the point system to determine if an additional caregiver is needed.

From approximately 9:45am to 10:00am, the assistant was caring for 10 children alone totally 26 points while the provider went to the store.
Plan of Correction: I am considering finding a backup assistant who meets the required standards but if one caregiver is out, both adults in the daycare must meet the qualifications.

Topical Skin products do not need to be kept locked, but shall be inaccessible to children.

Three diaper creams were on the floor near where children play.
Plan of Correction: Diaper creams already moved in kid tote bag. Bag put in cabinet with locks.
