During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site renewal/ inspection was initiated and completed on 7/02/2025, as a part of the provisional licensure period. The on-site inspection began at 9:11am and ended at 10:20am. The inspector reviewed compliance in the areas listed above. There were five children present and two caregivers; the point total was 10. The inspector reviewed five children?s records and two caregiver/household members records on-site on 7/02/2025.
This inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 7/09/2025. A POC submitted after this date will not appear on the public website.

Mattresses, when used, shall be covered with a waterproof material that can be cleaned and sanitized.

Two cribs mattresses were not covered with a waterproof material that can be cleaned and sanitized.
Plan of Correction: I'll follow the regulations and buy two additional waterproof mattresses.
Standard 8VAC20-800-80-C
The provider shall document in the child's record the method of verification of the child's age and identity; and the names and locations of the previous child care programs and
schools the child has attended.

Child #3's file was missing the birth certificate number, date issued, date viewed and person viewing the document and had been attending over 3 months.

Child #4's file was missing documentation of locations of the previous child care programs attended and has been attending for over 7 years..
Plan of Correction: The papers of child #3 have already filled in the information. The records of child #4 has also been completed.
