Inspector Notes
An annual subsidy inspection was conducted on 8/3/2022. At the time of the inspection, there were five (5) children in care. Children were observed interacting with the provider and napping. The sample size was three children's records and the provider and household member records. Areas of non compliance are identified on the violation notice and were discussed with the provider at the conclusion of this inspection.

Based on record review nd interview it was determined the center did not obtain documentation that the child has been immunized according to the requirements of subsection A of ? 32.1-46 of the Code of Virginia and applicable State Board of Health regulations.
Evidence:
1. The records for child #1 and #2 did not contain documentation of immunizations.
2. Provider confirmed that they did not have immunization documentation for child #1 and #2.
