Inspector Notes
A Subsidy Health and Safety inspection was conducted on 04/22/2024 to review supplemental health and safety requirements. A licensing inspection was conducted on the same date as the SHSI supplemental inspection; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. The information gathered during the inspection determined no violations with applicable standards or law. No violations were issued.

Based on record review and interview it was determined the provider did not ensure that each child's record contained all of the required components.
Evidence:
1. The records for child #2, 3, & 4 did not contain proof of the child's identity.

Based on observation it was determined that the provider did not ensure that all hazardous substances were stored in areas that are in accessible to children.

1. There were cleaning chemicals unlocked in the bathroom that the children in care utilize.
2. Provider confirmed these items were unlocked.
