Inspector Notes

A renewal inspection was initiated on 11/2/2022 and concluded on 11/2/2022. There were nine children present with two staff supervising. The point total was 24. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and six HHM/ staff records were reviewed. The inspection started at 9:20 am and concluded at 11:40 am.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record review the provider failed to have staff submit to a fingerprint background check prior to the first day of employment.
Evidence:
Staff #2 was hired on 9/19/22 and started working with the children on 10/12//22. The fingerprint background check is dated 10/17/2022.
Plan of Correction: The provider will ensure that when a staff person is hired the fingerprint criminal history check is completed prior to the staff person working with children.
Standard 22.1-289.036-B-4
Based on record review and interviews the provider failed to obtain a copy of the results of a sex offender registry check, and a search of the child abuse and neglect registry or equivalent registry from any state in which the individual has resided in the preceding five years, by the end of 30th day of moving in.
Evidence:
HHM #2 moved in May, 2022. HHM #2 lived outside of the state of Virginia in the past five years. HHM does not have an out-of-state background check from that particular state.
Plan of Correction: The provider will submit the out of state background checks for the appropriate state.
Standard 8VAC20-770-40-D-4-a
Based on record review and interviews the provider failed to ensure that a central registry check was conducted when a person 18 years or older begins residing in the home.
Evidence:
1. Household member #2 began residing in the home in May, 2022. The provider does not have a Central Registry check on HHM #2.
Plan of Correction: Provider completed the form and put it on the portal 5/18/22. The provider realized that she had not submitted the search request. The search request was submitted in September 19, 2022.
