Inspector Notes

A self reported monitoring inspection was initiated on 02/15/2022 and concluded on 02/24/2022 in response to a self reported incident regarding a serious injury that occurred at the center on 02/09/2022. The inspector conducted an unannounced inspection on-site on 02/15/2022 from approximately 11:48am to 12:25pm. Interviews with staff were completed and observations were made at the center. Additional interviews were conducted off-site on 02/15/2022. The evidence gathered during the investigation did not support non-compliance with licensing standards. There are no violations resulting from this inspection.
