Plan of Correction: In response to the 8VAC20-780-(2)-150-B violation, the form was given back to the family for proper signage and date.
Center Director and Assistant Director will remind families to obtain the proper signage and date on all immunization records. Center Director and Assistant Director will review all immunization records upon receipt for full completion including proper signature and date and will immediately return any without proper signage and dates to the family.

The center shall ensure that areas and equipment of the center, inside and outside, are maintained in a safe and operable condition.
1. Outside outlet cover broken and exposing the outlet to children on playground.
2. Peeling wallboard in sink area in Early Preschool 2 classroom and electrical wall plate next to exterior door shattered with rough edges.
3. Peeling plastic on the toy drum head located in the inside play area.

Plan of Correction: In response to the 8VAC20-780-(2)-70 violation, the Assistant Director immediately placed a work order for the broken and exposed outside outlet cover, the rough edged electrical wall plate next to the exterior door in Early Preschool 2, as well as the peeling wallboard in the sink area in the Early Preschool 2 classroom. The work order was completed on November 19, 2025. The toy drum head with the peeling plastic located in the inside play area was immediately discarded.
Going forward, the Center Director and Assistant Director will inspect all areas of the center for any hazards during opening and closing rounds of the center. Any hazards found will be addressed at that time. The Center Director and Assistant Director will also ensure that all classrooms are conducting playground inspections by completing the Playground Checklists weekly and rectifying any issues noted.

Disposable diapers shall be disposed of in a manner that neither the staff member?s hand nor the soiled diaper touches the exterior surface on the disposal can. A staff member in the Toddler room was observed changing a diaper and dispose of the diaper in a trash can that required the staff to touch and lift the lid to dispose of the soiled diaper.
The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use. A staff member in the Toddler room cleaned the diaper changing surface but did not sanitize the surface.

Plan of Correction: In response to the 8VAC20-780-(7)-500-B violation, the trash can used to dispose of the soiled diaper is hands free as it has a step, however, the staff member failed to use the step. The staff member was retrained on November 17, 2025, the same day as the inspection, on the diaper changing procedure and policy.
Center Director and Assistant Director will review and retrain all staff on the diaper changing procedure and policy and the cleaning and sanitization policy during the next staff meeting on December 11, 2025. Center Director and Assistant Director will have all staff sign and date the procedures and policies once re-training is completed. In addition, Center Director and Assistant Director will make routine observations of staff completing diaper changes and proper cleaning
and sanitization of surfaces to ensure continued compliance and understanding.
