Inspector Notes

A renewal inspection was initiated and concluded on 1/20/2023. There were 12 children present with 2 providers supervising. The point total was 30. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 3 caregiver / household member records were reviewed. The inspection started at 12:10 pm and concluded at 2:00 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the licensee did not ensure that each enrolled child's record contains all of the required documentation.
Evidence:
Five children's records were reviewed.
The records for child #1 and child #2 did not contain addresses for the designated emergency contacts to be called in the event that a parent cannot be reached in an emergency.
The records for child #1 and Child #2 did not contain the Hospitalization plan number.
The licensee was unable to demonstrate that this information had been obtained for child #1 and child #2.
Plan of Correction: The licensee stated that she would request this information from the parent today at pick up time and have this documentation in the child's record by 1/23/2023.
