A subsidy health and safety (SHSI) inspection was completed. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.
Information gathered during the inspection determined no violations with applicable standards or law.

Based on a review of records and interview, the vendor did not ensure that each caregiver includes int heir annual training, the department's health and safety update course.
Evidence:
Three caregiver / household member records were reviewed.
The record for caregiver #1 did not contain documentation to demonstrate that the department's health and safety update course had been completed within the last 12 months.
When asked, caregiver #1 stated that she had not taken the annual course update in the past 12 months.

Plan of Correction: The vendor stated that she will begin completing the 3 hour class this week with the intent to complete it within 7 days.

Based on a review of the family day home's Emergency Plan and interview, the Family Day Home's written emergency plan does not include procedures for Lockdown.
Evidence:
1. The written emergency plan did not address lockdown procedures.
2. The licensee confirmed that she used a model form which she did not realize was missing elements required of susbsidy vendors and that the current written plan did not contain lockdown procedures.
