A subsidy health and safety (SHSI) inspection was completed on 9/26/2023. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

** 2nd Repeat Violation ** Based on a review of records and interview, the vendor did not ensure that each caregiver includes in their annual training, the department's annual health and safety update course. Evidence: Three caregiver / household member records were reviewed. The record for caregiver #1 did not contain documentation to demonstrate that the department's health and safety update course had been completed within the last 12 months. When asked, caregiver #1 stated that she had not taken the annual course update in the past 12 months.
Plan of Correction: The licensee will complete this training within 30 days and forward a copy of the training certificate to the inspector upon receipt.
