Inspector Notes
A monitoring inspection was initiated and concluded on 5/6/2024. There were 5 children present, ages 2 to 5 years old, with the provider and an assistant supervising. The point total was10. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. Children were observed having an afternoon snack, playing with toys and being picked up by parents. A total of 5 child records and 3 Provider / Assistant / Household Member records were reviewed. The inspection started at 4:00 pm and concluded at 5:55 pm.
Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Repeat violations were found and are identified within this report.
Violations
2Evidence:
The record for child #4 contained an immunization record and physical form which were not signed by a licensed physician, the physician's designee, or an official of a local health department.
A copy of the signed documentation will be placed in Child #4's record.
Based on a review of records and interview, the licesee did nt ensure that each child's record contains all of the required information to include the Name, Address and phone number for 2 people to be contacted in the case of an emergency if the child's parent cannot be reached.
Evidence:
Five children's records were reviewed.
The record for child #2 (enrolled 8/2/2021) did not contain the address for one of two of the listed emergency contacts.
The record for child #5 (enrolled 12/29/2021) did not contain the address for either of the two listed emergency contacts.
The licensee confirmed that these addresses were no listed in the children's records.
