Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on August 27, 2024. The on-site inspection began at 3:30 pm and ended at 4:40 pm. The inspector reviewed compliance in the areas listed above. CDC: There were 5 children present and 3 caregivers. The inspector reviewed 5 children?s records and 2 staff records on-site. This inspection included document review, tour of the facility, interviews and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

(Repeat Violation) Based on review, provider did not ensure 4 of 5 child records have documentation of required information.

Evidence:
The record for child #2 (start date 12/11/2023) did not have insurance plan number
The record for child #3 (start date 1/9/2023) did not have insurance plan number and emergency contact addresses.
The record for child #4 (start date 1/25/2021) did not have insurance information.
The record for child #5 (start date 9/6/2022) did not have the address for the second emergency contact.
Plan of Correction: Provider will request information from parents and information will be added to each child's record.
