Inspector Notes

A complaint inspection was initiated on May 4, 2023 and completed on June 29, 2023 regarding concerns of staffing and supervision and programs. During the inspection, the inspector conducted Interviews with staff and collaterals, observations were made in the center, and documentation was reviewed. The preponderance of the evidence gathered during the investigation does not support the allegation; therefore, the complaint is determined to be not valid.
