Standard 22.1-289.035-B-2
Providers must obtain a completed national criminal background check prior to the employee's first day of employment. Staff #3, completed national criminal background check, was dated seven days after the employees first day of employment.
Plan of Correction: Since this happened in 3/24 we started a new protocol in 10/24 to send it in on the 1st day they have started.
Standard 22.1-289.035-B-4
(Repeat Violation) The center is required to obtain background checks from any state in which the individual has resided in the preceding five years. The center did not request a search of the child abuse and neglect registry from three separate states, for Staff #7, who has been employed for three months.
Plan of Correction: Will insure to get both dept of background checks (criminal & abuse) if not under one search form. Also printed new list of state's websites. New forms have been sent out.
Standard 8VAC20-780-160-A
Documentation of a negative tuberculosis (TB) screening must be submitted at the time of employment, before coming into contact with children, and shall have been completed within the last 30 days of the date of employment. Staff #7 TB was dated twelve days after the date of employment, Staff #9 TB was dated nine days after the date of employment, and Staff #10 TB was dated thee days after the date of employment.
Plan of Correction: When a new hire says yes to the job, we will ask when was their last TB test; if need be, we will send them for a new one before their 1st day.
