Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A monitoring inspection was initiated on 12/09/2021 and concluded on 12/27/2021. The director was contacted by email to initiate the inspection. There were 33 children present and 8 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 3 children?s records and 2 staff records, along with any requested program records submitted by the facility to determine if required documentation was complete.

New requirements became effective on October 13, 2021. The facility has not yet complied with the following requirements: written procedures for prevention of shaken baby, abusive head trauma, coping with crying babies, safe sleeping practices and SIDS awareness. The facility is to review the new requirements and work with their assigned inspector to ensure future compliance.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review of one medication, the center failed to ensure that all medication was returned to the parent after the 14 days past the authorization date.

Evidence:
1. Child # 4 medication's authorization was completed on 9/23/2021 and was signed by the parent. The authorization is only good for 14 days with out a physician's signature. The medication was at the center on the day of the inspection.
Plan of Correction: The director will send the medication home today. All authorizations and medications will be monitored.
