Inspector Notes

A monitoring inspection was conducted on September 7, 2022. There were 95 children present, with 15 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. The children were observed playing outside, writing, and engaged in tabletop activities. A total of 5 child records and 7 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Standard 22.1-289.035-B-4
Based on review of staff records, the center did not obtain a copy of the results of a criminal history check and or child abuse and neglect registry search from any state in which an individual has resided in the preceding five years.
Evidence
Staff # 3 (Date of hire 8/1/22) has resided in Texas and Maryland in the preceding five years. The center did not obtain a criminal history check for Texas before first day of employment or a child abuse and neglect registry search for Texas and Maryland within 45 days of first day of employment.
Plan of Correction: We will do the out of state background checks and send the results to our inspector. Going forward we will make sure to conduct all background checks within the required timeframe.
Standard 8VAC20-780-70
Based on review of staff records, one did not contain documentation that two or more references as to character and reputation as well as competency were checked before employment.
Evidence:
The record for staff #1 had documentation that only one reference was checked before the first day of employment.
The record for staff #2 and #3 did not have documentation that any references were checked before the first day of employment.
Plan of Correction: We will do two reference checks before we hire a new employee.
