Violations

1
Standard 22VAC40-185-340-F
Based on staff interviews, the facility did not ensure that all children under 10 years of age always shall be within actual sight and sound supervision of staff.

Evidence: 1) At approximately 10:00 a.m. on May 21, 2021, Staff #1 walked Child #1 (age 5) to their classroom. Staff #1 reported she called out the child's name, left the child in the classroom, and walked back to the lobby. When Staff #1 was walking to the lobby, she passed Staff #2 in the hallway. Staff #2 went to the classroom and found that Child #1 was alone and unsupervised. Staff #1 acknowledged she did not check if there was anyone in the classroom when she left Child #1. It is estimated that Child #1 was out of sight and sound supervision of staff for approximately 1 to 2 minutes.
Plan of Correction: Per the Center: "When a teacher is leaving a classroom, always turn off the light. When a staff member drop [sic] a child in a classroom, eye contact between the 2 adults must be made."
